I/we, as an adult participant or as parent(s) or legal guardian(s) for registered minor (hereinafter referred to as "Participant"), hereby give permission for Participant to participate at High 5 Inc. and all activities associated therewith. I/we further authorize any High 5 counselor, chaperone, coach, or other staff member in his/her discretion, to obtain medical or emergency treatment for Participant. In consideration for Participant's participation, and for the use of the facilities and property, I/we further, as parent(s) or legal guardian(s) of Participant (a minor), on behalf of said Participant as well as on the behalf of myself/ourselves, hereby forever release, acquit, discharge, and hold harmless High 5 Inc., any officers, employees, agents, counselors, chaperones, coaches, helpers, aids, or assistants, parent or subsidiary companies of the above, of and from any and all liability, claims, actions, causes of action, lawsuits, or rights or claims for damages, including but expressly not limited to, claims for High 5's own negligent acts or omissions, relating to or in any way arising out of Participant's participation at High 5. I hereby grant permission to High 5 to photograph participants for publication in
connection with public relations. I release High 5 from any claims that may arise or be based upon such publications. I understand that High 5 may use the photos indefinitely. I/we have read the information sheet regarding rules and fees and have kept a copy of the sheet for future reference. I/we have read and understand the above and enter into same of My /Our own free will.
Guest fees are a one day, one-time use. Guest fees must be paid the day the guest is coming to High 5. Guest passes are limited to two usages per guest. After using two guest passes, the guest must become a member to continue using the facility. Guests are required to adhere to the facility's code of conduct and general rules and policies. If you have not prepaid the guest fee, and Member Services is not open, guest privileges are disallowed. In the event of inclement weather which prevents 50% or more of usage time, the guest can see Member Services to get a replacement guest pass to be used within thirty days. For tennis court usage, guest must be accompanied by a member. Guests must provide proof of payment PRIOR to using the courts. Visiting players for USTA matches are exempt from paying guest fees. Ultimate Tennis and third party ladder organizations, e.g. MP Tennis Ladder, are NOT exempt from guest fee payments.
High 5 is committed to providing a welcoming environment for all of our members and guests. To ensure safety and comfort for all, we require individuals to act appropriately at all times when using the facility or participating in any of our memberships, programs, classes, events, leagues or activities. All members and guests are required to read and sign the High 5 Waiver form prior to using the facility for the first time. In addition, members must be current and in good standing prior to using High 5’s facility. We require all participants coming to High 5 to behave in a mature and responsible way and to respect the rights and dignity of others. Our code of conduct does not permit language or actions that can hurt, threaten, frighten or belittle another person or that falls below a generally accepted standard of conduct. Abusive, disrespectful, profane, or otherwise inappropriate language or behavior is unacceptable and will NOT be tolerated. Immediate suspension of membership privileges will be imposed at High 5's discretion. Only approved service animals are permitted in the facility. No other pets or animals are allowed including comfort animals. No smoking or vaping in High 5, to include the parking lot.
Tennis and pickleball court use is for members in good standing, their paid guests and tennis and pickle ball program participants ONLY. Court shoes and shirts required on court at all times. Coaching/lessons permitted ONLY from High 5 coaching staff. Staff reserves the right to rearrange court assignments as necessary. High 5 is a multisport & multi-activity facility. Noise related to these activities, such as music, children playing, coaching, etc., is not inevitable, it is to be expected. In the event of inclement weather, the court attendant or maintenance staff will determine if and when courts are playable. Court use is NOT permitted unless cleared for play by the court attendant or High 5 staff.
